Forget-Me-Not Baskets is partnering with Salem Cares Foundation through Salem Stones to gift memorials to those facing financial challenges.
Forget-Me-Not Baskets’ mission is providing grief support, comfort and remembrance to families experiencing loss, and this partnership with Salem Cares is a beautiful extension of our mission. Together, Forget-Me-Not Baskets and Salem Cares will be offering cemetery monuments/memorials to those who have experienced or are experiencing financial hardship. This gift will allow parents to honor their child and provide solace to grieving families.

Please fill out the application below. Filling out the application does not guarantee you will receive a monument/memorial. Once your application has been reviewed, you will receive a notice from Forget-Me-Not Baskets. Due to the amount of applications we receive, it could be 4-6 weeks before we are able to review.

If your application is approved, Forget-Me-Not Baskets will reach out to you with steps to move forward. You will have multiple templates to choose from to personalize your gifted monument/memorial. Salem Cares will process and ship your monument at no cost. Timeframe for delivery is usually 60-90 days after the order has been submitted.
